A crisis continues to brew within the pharmaceutical research and development (R&D) enterprise: productivity continues declining as costs rise, despite ongoing, often dramatic scientific and technical advances. To reverse this trend, we offer various suggestions for both the expansion and broader adoption of modeling and simulation (M&S) methods. We suggest strategies and scenarios intended to enable new M&S use cases that directly engage R&D knowledge generation and build actionable mechanistic insight, thereby opening the door to enhanced productivity. What M&S requirements (...) must be satisfied to access and open the door, and begin reversing the productivity decline? Can current methods and tools fulfill the requirements, or are new methods necessary? We draw on the relevant, recent literature to provide and explore answers. In so doing, we identify essential, key roles for agent-based and other methods. We assemble a list of requirements necessary for M&S to meet the diverse needs distilled from a collection of research, review, and opinion articles. We argue that to realize its full potential, M&S should be actualized within a larger information technology framework—a dynamic knowledge repository—wherein models of various types execute, evolve, and increase in accuracy over time. We offer some details of the issues that must be addressed for such a repository to accrue the capabilities needed to reverse the productivity decline. (shrink)

The philosophy of patient-centred care has become widely embraced but its implementation is dependent on interrelated factors. A factor that has received limited attention is the role of policy tools. In Ontario, one method government can use to promote healthcare priorities is through health regulatory colleges, which set the standard of practice for health professionals. The degree to which government policy in support of patient-centered care has influenced the direction provided by health regulatory colleges to their members, and ultimately impacted (...) actual patient care, remains unclear. This study investigates the extent to which Ontario’s health regulatory colleges have provided explicit written guidance to members related to the importance of patient-centred care. It also explores applied and theoretical explanations that may further our understanding of why patient-centred care has not been more fully embraced. Findings reveal that guidance provided by Ontario’s health regulatory colleges varies widely. Institutional barriers and the choice of policy tools for disseminating government preferences may hinder full implementation of the principles of patient-centred care. More fully understanding the role health regulatory colleges’ play in facilitating the implementation of health policy will contribute positively to dialogue and to efforts to achieve positive health system reforms. The integrative memory model combines five core memory systems with an attributional system. We agree with Bastin et al. that this melding is the most novel aspect of the model. But we await further evidence that the model's substantial complexity informs our understanding of false memories or of the development of recollection and familiarity.

The binary remember/know task requires participants to dichotomize their subjective recognition experiences into those with recollection and those only with familiarity. Many variables have produced dissociative effects on remember/know judgments. In contrast, having participants make independent recollection/familiarity ratings has consistently produced parallel effects, suggesting the dissociations may be artifacts of using binary judgments. Bodner and Lindsay reported a test-list context effect with binary judgments: Increased remembering but decreased knowing for a set of critical items tested with a set of less-memorable (...) items. Here we report a parallel effect of test-list context on recollection and familiarity ratings, induced by a shift in response bias. We argue that independent ratings are preferable to binary judgments because they allow participants to directly report the co-occurrence of recollection and familiarity for each item. Implications for the measurement of self-reported recognition experiences, and for accounts of recognition memory, are discussed. (shrink)

In the Deese-Roediger/McDermott paradigm, distinctive encoding of list items typically reduces false recognition of critical lures relative to a read-only control. This reduction can be due to enhanced item-specific processing, reduced relational processing, and/or increased test-based monitoring. However, it is unclear whether distinctive encoding reduces false recognition in a selective or global manner. To examine this question, participants studied DRM lists using a distinctive item-specific anagram generation task and then completed a recognition test which included both DRM critical lures and (...) either strongly related lures or weakly related lures. Compared to a read-control group, the generate groups showed increased correct recognition and decreased false recognition of all lure types. We then estimated the separate contributions of encoding and retrieval processes using signal-detection indices. Generation improved correct recognition by both increasing encoding of memory information for list words and by increasing memory monitoring at test. Generation reduced false recognition by reducing the encoding of memory information and by increasing memory monitoring at test. The reduction in false recognition was equivalent for critical lures and related lures, indicating that generation globally reduces the encoding of related non-presented items at study, while globally increasing list-theme-based monitoring at test. Purpose The purpose of this paper is to investigate perceived stressors and coping behaviours in student nurses on a pre-registration programme of study. Stress in student nurses has been identified with decreased emotional well-being and poor academic achievement. The significance of stress and coping behaviours in students during training has implications for education and practice. Design/methodology/approach The present study recruited 87 pre-registration student nurses in a cross-sectional design. Bivariate and multivariate analyses assessed the differences in field and year of study (...) and the students’ perceived stress and coping behaviours. Findings The findings showed that stress is a significant issue in nurse training. Fifty-three per cent of the participants had levels higher than the mean. Interestingly, the present study found that high-perceived stress was associated with avoidance behaviours. The most common type of perceived stress and ranked by highest factor were from written assignments and a lack of professional skills and knowledge. Their peer group and everyday life activities were shown as potential ways of coping with stressors. Thus, it seems reasonable to focus support on decreasing avoidant and enhancing stress-reducing behaviours. Practical implications Psychological stress and coping behaviours must be considered together, as perceived stress is bound by the ability to ameliorate stress by managing helpful and unhelpful behaviours. Originality/value The findings may suggest that a potential benefit could come from the provision of helpful strategies such as peer group support and reduction of avoidant behaviours. Also, there seems to be a need for greater mental health literacy in dealing with stress during training. (shrink)

"Resilience in the academic setting has been named as academic resilience and been defined as an increased probability of academic success despite stressful events and conditions" [Mitchell, 2021] p1474 based on the Academic Resilience Scale (ARS‐30) [Cassidy, 2016].

Most elderly patients die with an order in place that they not be given cardiopulmonary resuscitation . Surveys have shown that many elderly in different parts of the world want to be resuscitated, but may lack knowledge about the specifics of cardiopulmonary resuscitation . Data from countries other than the US is limited, but differences in physician and patient opinions by nationality regarding CPR do exist. Physicians’ own preferences for CPR may predominate in the DNR decision making process for their (...) patients, and many physicians may not want the participation of the elderly or believe that it is necessary. More complete and earlier discussions of a wider range of options of care for patients at the end of life have been advocated. The process ought to include education for patients about the process and efficacy of CPR, and for physicians on how to consider the values and levels of knowledge of their patients, whose preferences may differ from their own. Through the process of domestication the genetic make-up of farm animals can be changed by means of either selective breeding or genetic engineering. This paper is about the ethical limits to such genetic changes. It is suggested that the ethical significance of domestication has become clear recently in the light of genetic engineering, but that the problem has been there all along. Two ethical approaches to domestication are presented, genetic integrity and animal welfare. It is argued that the welfare approach (...) is superior. Abela accepts Fine’s account of realism and instrumentalism, but thinks that we can reject the Natural Ontological Attitude by distinguishing the theoretical attempt to make sense of scientific practice from choosing the attitude we bring to the debate, or to science itself. But Abela’s attitudes are vulnerable to Fine’s criticisms of the philosophical positions. However, if we take attitude as contrastive and as full‐blooded enough to lead to different behaviour we can see a gap in Fine’s position. He cannot tell (...) us why it is science that he trusts. When we look more widely, the philosophical positions might be seen as ways of justifying that trust. (shrink)

As the technology for ectogenesis continues to advance, the ethical implications of such developments should be thoroughly and proactively explored. The possibility of full ectogenesis remains hypothetical at present, and myriad concerns regarding the safety and efficacy of the technology must be evaluated and addressed, while pressing moral considerations should be fully deliberated. However, it is conceivable that the technology may become sufficiently well established in the future and that eventually full ectogenesis might be deemed ethically acceptable as a reproductive (...) alternative to gestation within a human womb under certain circumstances. If the safety and efficacy of full ectogenesis are established, if ethical dilemmas are sufficiently well addressed, and if the technology is offered as a reproductive option to cisgender heterosexual individuals or couples desiring to become parents, there is a moral obligation grounded in social justice to ensure that full ectogenesis be made available to individuals or couples identifying as members of sexual‐ or gender‐minority groups who likewise seek to pursue parenthood. We examine the history of access to current family‐building options, including assisted reproductive technology, surrogacy and adoption, for these populations and conclude that in the absence of robust empirical evidence suggesting an increased risk of harm to children of individuals and couples who identify as members of sexual‐ or gender‐minority groups, equitable access to ectogenesis as a pathway to parenthood for sexual and gender minorities must be assured as a matter of reproductive justice.
